Lower eyelid surgery (blepharoplasty)

Lower eyelid plastic surgery is a surgical procedure that aims to improve the appearance of the lower eyelids and reduce the functional problems associated with them. Indications for this procedure include:

1.   Excessive loose skin on the lower eyelids, causing puffiness or wrinkles.
2.   Fat bags under the eyes that cause swelling and change the shape of the eyelids.
3.   Lack of elasticity and tension in the skin of the lower eyelids, which leads to loss of definition of the eyelid lines.

 

Lower eyelid surgery can be performed under local or general anesthesia. There are two main methods of operating the lower eyelids depending on the patient's anatomy:

1.   Transconjunctival method - the cut is made in the conjunctival sac, from the inner surface of the eyelid (it is invisible)
2.   Percutaneous method - the cut is made 2mm below the eyelashes on the lower eyelid (after healing it looks like a typical mimic wrinkle)

 

Qualification for the procedure requires a comprehensive assessment of the eyelids and adjacent structures. The percutaneous method is indicated in the case of significant flaccidity of the skin around the eyelids. In addition, this method can be combined with the midface lift procedure - i.e. pulling up the middle part of the face.
If the patient has a weakening of the structures supporting the lower eyelid, the plastic surgeon performs the cantopexy procedure - i.e. lining and strengthening the structures of the tarsus, i.e. the eyelid skeleton.

 

The treatment begins with careful marking of the cutting lines, which are carefully planned to minimize visible scars - the cut is made under the eyelash line. Then, precise preparation is performed in the designated areas, removing excess skin, modeling fatty tissue and/or muscles to achieve the desired shape of the eyelid. After the procedure, the incisions are sutured to minimize scarring. The sutures are removed after about 7-8 days

Lower eyelid plastic surgery can be combined with laser therapy in this area and the administration of the patient's autologous fat (nanofat) in order to improve the condition of the eyelid skin and fill in unevenness.

 

After lower eyelid surgery, it is important to follow the exact instructions of your plastic surgeon to ensure an optimal recovery process. The following are general recommendations:

1.   Use winter eyelid compresses to reduce swelling and redness for a few days after the procedure.
2.   Avoid strenuous physical activity for about 4 weeks after the procedure.
3.   Use the prescribed analgesic therapy and anti-inflammatory drugs, if recommended by the surgeon.
4.   Protect from direct sunlight and use sunscreen creams with a high SPF on treated areas.
5.   Avoid wearing contact lenses and eyelid makeup for several days after the procedure.


After recovery from lower eyelid surgery, patients typically experience an improvement in both eyelid aesthetics and function. The expected effects are:

1.   Smoothing wrinkles and reducing swelling on the lower eyelids, giving the face a more youthful and rested look.
2.   Removal of fat bags under the eyes, which contribute to reducing swelling and changing the shape of the eyelids.
3.   Reducing the cut-off between the cheek and the lower eyelid
4.   Rebuilding the elasticity and tension of the skin of the lower eyelids, restoring the natural definition and proportions.
