Open vs closed rhinoplasty: which technique is more precise?

People who want to change the shape of their nose usually consider the decision to undergo surgery for a long time. For some, it is a matter of aesthetics; for others, it is a way to improve breathing comfort. Regardless of the reason, it is worth knowing that the surgeon may offer two methods: open or closed. They differ in technique, healing time and scope of possibilities. This article will help you understand what both techniques involve and which of them may better meet your needs.

Rhinoplasty: what is this treatment and when is it worth deciding on it?

Rhinoplasty is a surgery that allows changing the shape and improving the function of the nose. It may help people who have problems with breathing due to a deviated septum, as well as those who want to improve the appearance of the nose – its size, width and tip. The treatment is usually performed under general anaesthesia.

It is not only a matter of aesthetics – although this aspect is most often what prompts patients to take action. Abnormal nose structure may cause snoring, chronic sinus inflammation or difficulties in daily functioning. Rhinoplasty is a treatment of both corrective and functional character.

It should be borne in mind that rhinoplasty changes something very visible – the central part of the face. Therefore, trust in the surgeon and awareness that the final result will be permanent are so important. Every nose is different, just as patients’ needs are different – therefore, consultation, individual selection of the method and having realistic expectations of the results are so important.

Closed rhinoplasty: a gentle approach without visible scars

Closed rhinoplasty is a technique in which the surgeon makes all incisions inside the nose. This means that no external scars remain. This is a solution for patients who require minor correction – e.g. removal of a hump, slight elevation of the tip or improvement of nostril symmetry.

The advantage of this method is less swelling, shorter operation time and faster return to daily activities. The patient can usually return to work after a few days, although full recovery takes longer. Patient comfort after the treatment may also be higher – less skin tension is felt, breathing through the nose is easier.

However, it is worth remembering that not every nose qualifies for such a treatment – limited access means that the surgeon has fewer correction possibilities. This technique may be less precise for more significant changes.

Closed rhinoplasty requires a high level of experience and precision from the surgeon. It involves working in a limited surgical field, often without direct visualisation of the nasal structures. Therefore, the decision to use this technique should be preceded by a thorough anatomical analysis and a surgical plan – it cannot be “adjusted on the fly”.

Open rhinoplasty: full control and greater possibilities

When more extensive reshaping of nasal structures is necessary, the surgeon may propose open rhinoplasty. In this technique, an additional incision is made on the nasal columella, which allows the skin to be fully elevated and provides a clear view of the cartilaginous and bony framework.

This method is particularly useful in secondary surgeries, asymmetries, reconstructions and corrections of the nasal tip. It is also frequently chosen for complex medical cases, e.g. after trauma. This option is suitable for those who expect precision and are not afraid of a longer healing process.

Although it involves a longer recovery period and the possibility of a small scar, it gives the surgeon full control over the shape of the nose. The patient gains greater confidence that the final result will correspond to the plan.

For many people, peace of mind and a sense of security are the most important factors. This technique also allows for more precise surgical planning using 3D imaging, which further increases the precision of the operation and allows you to “see the future nose” before entering the operating theatre.

What does rhinoplasty involve and what should you know before the procedure?

In short: it is a procedure planned with great accuracy, performed in comfortable conditions and carried out by an experienced surgeon. Before the operation, a consultation with the doctor is necessary – expectations, health status and tests must be discussed. The procedure itself lasts from two to three hours and is performed under general anaesthesia. After the operation, the patient remains under observation in the clinic, usually for 24 hours.

After the procedure, a plaster cast and silicone splints are placed inside the nose. For several days, rest is recommended, along with avoiding bending over, physical exertion and wearing glasses. Swelling subsides gradually, and the final result is visible one year after the operation.

It is worth preparing mentally – the face looks different after surgery, with swelling, bruising, and changes in facial expression. This is normal and temporary. The key to success is patience and strict adherence to the doctor’s recommendations, including follow-up appointments.

Cold compresses, sleeping with the head elevated, and avoiding sun exposure may also be helpful. Some patients also use lymphatic facial massage after a few weeks – always with prior approval from the doctor.

Which technique gives better results? Open or closed rhinoplasty?

There is no single answer. The choice depends on the type of nasal deformity, the scope of the surgery, the history of previous procedures, and the patient’s preferences. Open rhinoplasty offers greater possibilities in difficult cases and allows for very precise results. Closed rhinoplasty, on the other hand, is a good option for those who need minor corrections and value a quick recovery.

Each case should be analysed individually. The surgeon takes into account the structure of the nose, skin thickness, cartilage arrangement, and the patient’s aesthetic expectations. Increasingly, the decision is made with the help of computer visualisation of the result – a tool that enhances comfort in planning the procedure for both the doctor and the patient.

It is worth talking, asking questions, and addressing doubts – because a well-performed rhinoplasty is not only about changing appearance but often also about improving quality of life.
